Required Student Documentation to include with the Application

1. Birth certificate
2. Current physical exam and immunization record (must be fully immunized as required by the State of Delaware. Please note that we do not accept religious exemptions)
3. Baptismal certificate (if applicable)
4. Last two years of report cards and standardized test scores for grades 2-8 (Kindergarten-most recent report card and standardized test score)
5. Psycho-educational evaluation (if applicable)
