- Birth certificate
- Current physical exam and immunization record (must be fully immunized as required by the State of Delaware)
- Baptismal certificate (if applicable)
- Most recent report card and standardized test scores
- Psycho-educational evaluation (if applicable)
- Teacher recommendation form
